If you would like a printout of your completed submission, FIRST click the Printer-friendly version at the bottom of this page. After completing the printer-friendly form, use your browser "print" function (e.g., Ctrl-P for Internet Explorer) for your printout, and then click the Submit button.

Once your application is submitted, you will receive an automatic e-mail message notifying you that your application has been received.

Build-Out Date

The date on which the system was constructed and placed into operation.

FRN—FCC Registration Number

An FRN, or FCC registration number, is a 10-digit number that is assigned to a business or individual registering with the FCC. This unique FRN is used to identify the registrant's business dealings with the FCC. The FCC will use the FRN to determine if all of a registrant's fees have been paid.

EIN—Employer Identification Number

EIN stands for Employer Identification Number. An EIN is a nine-digit number that the Internal Revenue Service (IRS) requires of all businesses and other employers to identify their tax accounts with the IRS. The EIN is one of two (2) identification numbers assigned by the IRS which are known collectively as Taxpayer Identification Numbers (TIN). The other identifier is the Social Security Number (SSN).
